摘要Objectives: Chronic sialadenitis secondary to salivary gland obstruction is common. Here we sought to evaluate the quality of life outcomes following gland preserving surgery with sialendoscopy for chronic sialadenitis in Hong Kong.
Materials and methods: A cross sectional survey of a retrospective cohort of both post-treatment and pre-treatment patients was performed. Post-treatment patients were identified as having undergone sialendoscopy for chronic sialadenitis at a tertiary, academic salivary referral center between May 2006 and December 2015. Pre-treatment patients were identified awaiting sialendoscopy. A Chinese questionnaire and modified Chinese version of the Oral Health Impact profile-14 (OHIP-14) were used to survey salivary symptoms. The study was approved by the Joint Chinese University of Hong Kong – New Territories East Cluster Clinical Research Ethics Committee. Statistical analysis was performed with SPSS 20.0.
Results: One hundred and forty seven patients were identified. Ninety four patients completed the survey with a response rate of 63.9%. 41 (43.6%) of the patients were male and 53 (56.4%) were female. Mean and median age was 49.5 and 51.0 years old respectively, interquartile range 40.0 to 59.0 years old. The majority of cases involved the parotid gland in 52 (55.3%) of patients. The median total OHIP scores in pre-treatment patients were 17.0 (Interquartile range 9.0 - 26.0) and in post-treatment patients were 8.0 (Interquartile range 4.0 - 14.25). Mann Whitney U test showed significantly better scores post treatment in OHIP subscales of functional limitation (p = 0.02, effect size -0.24), physical pain (p <0.001, -0.40), physical disability (p = 0.02, effect size -0.25), psychological disability (p = 0.07, effect size -0.28) and handicap (p = 0.008, effect size -0.27). OHIP total scores were also significantly improved post treatment (p = 0.003, effect size -0.31). Seven (8.6%) of patients in the post-treatment group needed a gland excision. Complications occurred in 8 (9.9%) of patients including false track, infection and others.
Conclusion: Sialendoscopy for chronic sialadenitis is a safe procedure and in post-treatment patients shows a significantly improved quality of life compared to pre-treatment patients, indicating the role sialendoscopy has in the minimally invasive treatment of chronic sialadenitis.
