摘要:目的分析颌面颈部脂肪来源肿瘤的临床表现，病理特征及预后。方法选择2002年1月至2020年12月大连医科大学附属第一医院收治的病理诊断为脂肪来源肿瘤患者144例，其中男性93例，女性51例，平均年龄51岁，收集临床资料，对临床特征、影像学及病理特点进行总结和分析。术后随访3个月，6个月，1年，3年，5年，统计复发及再手术情况。结果本组患者发病部位以颈部最常见(68例)，额部及腮腺次之(各21例)，唇及颌骨少见。临床表现多为无痛性软质包块，部分肿瘤包膜完整；影像学表现为典型低密度灶，病理类型以脂肪瘤最为常见，共125例，占86%。术后复发4例(2.7%)。结论颌面颈部脂肪源性肿瘤好发于男性，病变主要集中于颈部、额部、腮腺及颊部。CT影像学检查具有较高诊断价值。外科手术是最主要的治疗方法，术后复发机率与病理类型相关。

Abstract:ObjectiveTo investigate the clinical manifestations, pathological features and prognosis of adipocytic tumors in maxillofacial and cervical region.MethodsThe clinical data of patients with adipocytic tumors, who were diagnosed pathologically in the Department of Oral and Maxillofacial Surgery at the First Affiliated Hospital of Dalian Medical University from January 2002 to December 2020, were collected. There were 93 males and 51 females, with an average age of 51 years. The clinical manifestations, imaging and pathological features and prognosis were analyzed. The patients were followed up at 3 months, 6 months, 1 year, 3 years and 5 years. The recurrence and reoperation were counted.ResultsThe most common site of disease was neck (68 cases), followed by forehead (21 cases) and parotid gland (21 cases). Tumors from lips and jaws were rare. Most of the clinical manifestations were painless soft mass, and some tumors had intact capsule. The imaging manifestations were typically low-density lesions. The most common pathological type was lipoma (125 cases, 86%). Postoperative recurrence occurred in 4 cases (2.7%).ConclusionMaxillofacial and cervical adipocytic tumors are common in men, and the majority of the lesions occur in neck, forehead, parotid gland and cheek. CT scans have great value in diagnosis. The most common pathological type is lipoma, and surgery is the main treatment. The recurrence rate is related to the pathological type.
