摘要:目的探讨数字化外科导板联合导板锁（guided cylinder with pin，GCP）预成临时修复体在种植即刻修复中应用的可行性。方法选取6例要求种植即刻修复的患者为研究对象。术前利用NobelClinician软件设计并制作数字化外科导板、导板联合GCP翻制石膏模型，预先制作临时修复体，择期手术并即刻修复。即刻修复当天，再次对患者进行CBCT拍摄，对比术前软件设计与实际种植体植入位置的偏差，并对临时修复体就位情况及患者满意程度进行评价。4~6个月待软硬组织稳定后行永久修复。其中对3例患者永久修复后利用工程逆向软件分析并量化术前设计与实际植入植体的三维偏差。结果实际种植位点与术前设计位点方向、角度等基本一致。3例已完成永久修复的患者共20个牙位，颊舌向平均偏差值< 0.3 mm，近远中偏差值< 0.2 mm，深度偏差值< 0.3 mm。患者术后当天经调改均顺利戴入预成临时修复体，达到了预期的修复效果。1例患者表示基本满意，占16.7%，5例患者非常满意，占83.3%。结论数字化外科导板联合GCP预成修复体用于种植即刻修复是可行的。预成修复体可为实现真正意义上的即刻修复提供参考。实际植体的三维位置与设计虽存在偏差但在可接受的误差范围之内。

Abstract:ObjectiveTo investigate the feasibility of prefabricated restoration made by the combination of digital surgical guide and guided cylinder with pin(GCP) in immediate implant restoration.MethodsSix patients requiring immediate repair were selected. Before the surgery, NobelClinician software was used to design and manufacture digital surgical guides.Plaster models were copy and made followed the GCP and surgical guides and then provisional restorations were made on the plaster model. Surgeries were performed on an elective basis and immediate restoration was performed after surgery. On the day of immediate restoration, CBCT was taken again to compare the deviation between the preoperative software design and the actual implant position. The placement of temporary prosthesis and the satisfaction of patients were evaluated.Permanent repair was performed after 4-6 months of healing.The reverse software was used to analyze the three-dimensional deviation between the preoperative design and the actual implanted implant in 3 cases.ResultsThe overall comparison showed that the direction and angle of the actual implant site were basically consistent with the preoperative design site. A total of 20 teeth were counted in 3 patients, the average deviation of buccolingual direction was less than 0.3 mm, mesial distal deviation was less than 0.2 mm, and depth deviation was less than 0.3 mm. The patients were successfully put into the temporary restoration after the operation, and the expected effect was achieved. One patient was basically satisfied (16.7%) and five patients were very satisfied (83.3%).ConclusionIt is feasible to prepare temporary prosthetic techniques for immediate repair afterimplantation.Prefabricated restoration provides a reference for real-time immediate restoration. The three-dimensional position of the actual implant is deviated from the design but is within acceptable limits.
