目的研究脱矿牙本质基质应用于拔牙后位点保存的骨改建情况。方法选取2019年1月至2020年6月就诊于南昌大学第四附属医院有意向种植牙齿的患者11例,共13个位点保存区域。行位点保存手术前(T0)、术后当日(T1)及术后6个月(T2)拍摄锥形束CT(cone beam CT,CBCT),分别测量近中骨高度、远中骨高度、颊侧骨高度、舌侧骨高度、牙槽嵴宽度及骨缺损深度。近远中骨高度、骨缺损深度的测量以近中邻牙近远中釉牙骨质界连线为基线(基线根方为?,冠方为+),颊舌侧骨高度、牙槽嵴宽度的测量以对颌牙咬合面为基线(基线根方为?,冠方为+)。比较T0、T1、T2时期位点保存区域的骨量变化,分析脱矿牙本质基质应用于位点保存的骨改变情况。结果T1较T0时,近中骨高度、远中骨高度增加,骨缺损深度减小,差异有统计学意义(P<0.05);颊侧骨高度增加,舌侧骨高度、牙槽嵴宽度减小,差异无统计学意义(P>0.05)。T2较T0时,近中骨高度增加,骨缺损深度、牙槽嵴宽度减小,差异有统计学意义(P<0.05);远中骨高度、颊侧骨高度、舌侧骨高度增加,差异无统计学意义(P>0.05)。T2较T1时,近中骨高度、远中骨高度、牙槽嵴宽度、骨缺损深度均减少,颊侧骨高度、舌侧骨高度增加,差异均无统计学意义(P>0.05)。结论脱矿牙本质基质应用于位点保存手术可获得良好的位点保存效果。

ObjectiveTo study the effect of demineralized dentin matrix on bone remodeling after tooth extraction.MethodsFrom January 2019 to June 2020, 11 patients with a total of 13 sites were selected from the Fourth Affiliated Hospital of Nanchang University. The site preservation surgeries were performed by the same doctor. Cone beam computed tomography (CBCT) was taken before site preservation surgery (T0), postoperatively on the day of surgery (T1) and 6 months after surgery (T2). The height of the proximal and distal bones, the height of the buccal bone, the height of the lingual bone, the width of the alveolar ridge and the depth of the bone defect were measured respectively. The measurements of the bone height and the depth of the bone defect of the proximal and distal enamel were based on the line between the bone boundary of the proximal and middle adjacent teeth (the root side from the baseline was ?, the crown side was +), and the measurements of the bone height and bone width of the buccal lingual side were based on the occlusal surface of the opposing teeth (the root side from the baseline was ?, the crown side was +). Changes of bone mass in the site preservation area among T0, T1 and T2 were compared, and changes of demineralized dentin matrix applied to the site preservation were analyzed.ResultsWhen T1 was compared to T0, the height of the proximal and distal bones was increased, and the depth of the bone defects was decreased. The differences between T1 and T0 were statistically significant (P<0.05). While the height of the buccal bone was increased, and the height of the lingual bone and the width of the alveolar ridge were decreased, no statistically significant differences were detected (P>0.05). When T2 was compared to T0, the height of the proximal and middle bones was increased, the depth of the bone defects, and the width of the alveolar ridge was decreased. The differences between T2 and T0 were statistically significant (P<0.05). While the height of the distal and middle bones, the height of the buccal bone, and the height of the lingual bone were increased, no statistically significant differences were found (P>0.05). When T2 was compared with T1, the height of proximal and distal bones, the width of alveolar ridge, and the depth of the bone defect were decreased, while the height of buccal bone and lingual bone were increased. The differences between T2 and T1 were not statistically significant (P>0.05).ConclusionApplication of demineralized dentin matrix in site preservation surgery can achieve good site preservation effect.
