摘要:目的探讨复发性抑郁障碍及双相抑郁障碍患者甲状腺激素水平的差异。方法选取符合《美国精神障碍诊断与统计手册第5版》（Diagnostic and Statistical Manual of Mental Disorders Fifth Edition，DSM-5）诊断标准的复发性抑郁障碍患者94例（复发性抑郁组），双相抑郁障碍患者97例（双相抑郁障组），收集患者一般资料，采用汉密尔顿抑郁量表17项（Hamilton Depression Scale-17 item， HAMD-17）评估患者抑郁严重程度，入组后次日晨抽取患者空腹静脉血检测两组患者的甲状腺激素水平。结果双相抑郁组的T4水平为（6.861±1.897）μg/dL，低于复发性抑郁组的（7.896±1.818） μg/dL （F=3.769，P=0.048）。 两组间T3、FT3、FT4、TSH水平比较，差异无统计学意义（P＞0.05）。两组中轻中度及重度患者的甲状腺激素水平均无明显差异（P＞0.05）。结论双相抑郁障碍患者较复发性抑郁障碍患者的血清T4水平低，可为早期识别双相情感障碍提供一定参考。

Abstract:ObjectiveTo study the difference of thyroid hormone levels in patients with recurrent depressive disorder and bipolar depression.MethodsAccording to the Diagnostic and Statistical Manual of Mental Disorders Fifth Edition (DSM-5) diagnostic standards, 94 patients with recurrent depression and 97 patients with bipolar depression were enrolled in the study, and the patients' general information was collected. Hamilton Depression Scale (HAMD-17) was used to evaluate the severity of patients' depression. Fasting venous blood was collected in the next morning after enrollment to determine thyroid hormone levels in the patients.ResultsThe level of T4 in patients with bipolar depression was (6.861±1.897) μg/dL, lower than that in patients with recurrent depression [(7.896±1.818) μg/dL,F=3.769,P=0.048]. There was no significant difference in T3, FT3, FT4, and TSH levels between the two groups (P＞0.05). There was no significant difference in thyroid hormone levels between mild-to-moderate and severe patients (P＞0.05).ConclusionSerum T4 level in patients with bipolar depression is lower than that in patients with recurrent depression, which is helpful for early diagnosis of bipolar disorder.
