摘要:目前临床上对于冠脉病变的主要介入治疗手段仍然是支架置入，但支架置入术后易出现冠脉支架内再狭窄(in-stent restenosis, ISR)及支架内血栓，同时术后长期的双重抗血小板治疗会增加出血风险等多种并发症。药物涂层球囊(drug-coated balloon, DCB)作为一种新兴的治疗手段，已经逐渐开展并应用于临床中，且在ISR的治疗上被多个指南作为ⅠA类证据推荐。如今越来越多的临床试验数据显示DCB在De novo血管病变、分叉病变及高出血风险患者中有着良好的治疗效果，同时在其他特殊冠脉病变，如弥漫、钙化、慢性完全闭塞病变中展示出新的应用前景，对于急性冠脉综合征患者及合并糖尿病患者的冠脉病变也有着潜在的治疗效果。本文将对DCB在冠脉病变中的应用展开综述。

Abstract:At present, the main interventional treatment for coronary artery disease is still stent implantation, while in-stent restenosis (ISR) and stent thrombosis are common after stent implantation. Long-term dual antiplatelet therapy could increase the risk of bleeding and other complications. Drug-coated balloon (DCB), which has been gradually developed and applied clinically as a promising way, is recommended as ⅠA evidence by international authoritative guidelines in the treatment of ISR. Nowadays, more and more clinical trials have shown that DCB has effect in de novo vessels, bifurcation lesions and patients with high bleeding risk. At the same time, there are widely application prospects in other special coronary lesions, such as diffuse lesions, calcification and chronic total occlusion lesions. It also has a potential therapeutic effect for patients with acute coronary syndrome or diabetes mellitus. This article will review the application of DCB in coronary artery disease.
