摘要:目的探究2型糖尿病(T2DM)患者血清促甲状腺激素(TSH)水平与内脏脂肪面积(VFA)的相关性。方法收集2021年6月至2022年12月纳入包头医学院第二附属医院标准化代谢疾病管理中心的300例T2DM患者,根据甲状腺功能分为甲状腺功能正常(EU)组185例和亚临床甲状腺功能减退(SCH)组115例。应用生物电阻抗方法测定VFA,并测定甲状腺功能及相关生化指标。采用Spearman相关分析探讨TSH水平与各变量的相关性,采用多元logistic回归分析T2DM患者发生SCH的影响因素。结果SCH组FPG、HbA1c、VFA、TG、LDL、SBP、DBP和UA水平高于EU组,HDL水平低于EU组,差异具有统计学意义(P<0.001)。2组间性别、年龄和糖尿病病程差异无统计学意义(P>0.05)。Spearman相关分析显示,血清TSH与VFA、FPG、HbA1c、TG、LDL、SBP、DBP和UA水平呈显著正相关(rs= 0.571、0.538、0.509、0.571、0.235、0.338、0.372、0.477,P<0.001),与HDL呈显著负相关(rs= ?0.439,P<0.001)。多元logistic回归分析显示,VFA是发生SCH的独立危险因素之一(P<0.001)。结论T2DM患者腹型肥胖与血清TSH水平相关。

Abstract:ObjectiveTo study the association between serum thyroid-stimulating hormone (TSH) and visceral fat area (VFA) in patients with type 2 diabetes mellitus (T2DM).MethodsTotally, 300 T2DM patients were enrolled in the study, who were admitted in the Metabolic Management Center at the Second Affiliated Hospital of Baotou Medical College from June 2021 to December 2022. According to thyroid function, T2DM patients were divided into euthyroid (EU) group 185 cases and subclinical hypothyroidism (SCH) group 115 cases. VFA, thyroid function, and biochemical indicators were measured using bioelectrical impedance. Spearman correlation analysis was used to analyze the correlation between TSH levels and various variables, and multiple logistic regression was used to analyze the influencing factors of SCH in T2DM patients.ResultsThe levels of FPG, HbA1c, VFA, TG, LDL, SBP, DBP, and UA in the SCH group were higher than those in the EU group, while the level of HDL was lower in the SCH group than that in the EU group (P<0.001). There were no significant differences in sex, age, and course of diabetes between the two groups (P>0.05). Spearman correlation analysis revealed that serum TSH was significantly positively correlated with VFA, FPG, HbA1c, TG, LDL, SBP, DBP, and UA levels (rs = 0.571, 0.538, 0.509, 0.571, 0.235, 0.338, 0.372, 0.477, P<0.001), whereas negatively correlated with HDL level (rs = ?0.439, P<0.001). Multivariate logistic regression analysis demonstrated that VFA was one of independent risk factors for SCH (P<0.001).ConclusionVFA in patients with T2DM are correlated with serum TSH level.
