摘要:目的分析不同牙龈黏膜厚度对Locator基台周围软组织状况的影响。方法收集2017年11月至2020年8月在大连市口腔医院就诊的下颌牙列缺失并采用Locator种植体修复的患者42例，根据种植体周围牙龈黏膜厚度分为4组：A组，黏膜厚度≤1.0 mm；B组，1.0 mm＜黏膜厚度≤2.0 mm；C组，2.0 mm＜黏膜厚度≤3.0 mm；D组，黏膜厚度＞3.0 mm。在义齿使用6、12个月后分别测量各组义齿基台周围菌斑指数（PLI）及牙周探诊出血（BOP）阳性检出率，并进行比较分析。结果（1）Locator附着体种植覆盖义齿使用6个月后，4组间PLI比较，差异无统计学意义（P＞0.05）；使用12个月后，4组间差异有统计学意义（P＜0.05）；D组 6个月与12个月比较，PLI差异有统计学意义（P＜0.05）。（2）义齿使用6个月和12个月后，4组间BOP阳性检出率比较，差异均有统计学意义（P＜0.05）；D组 6个月与12个月比较，BOP阳性检出率差异有统计学意义（P＜0.05）。结论当牙龈黏膜厚度＞3.0 mm，Locator基台周围软组织的PLI、BOP阳性检出率均增加。

Abstract:ObjectiveTo analyze the effect of different gingival mucosa thicknesses on soft tissue condition around Locator abutment.MethodsForty-two patients with edentulous mandibular dentition, who received treatment at Dalian Stomatological Hospital from November 2017 to August 2020, were included and divided into four groups based on the thickness of the gingival mucosa around the implant: group A (≤1.0 mm), group B (1.0 mm＜thickness≤2.0 mm), group C (2.0 mm＜thickness≤3.0 mm), and group D (＞3.0 mm). The plaque index (PLI) around the denture abutment and blood on Probing (BOP) positive detection rate in each group were measured and compared after 6 and 12 months of denture use.ResultsThere was no significant difference in PLI among the four groups after 6 months using locator attachment implant overdenture (P＞0.05). After 12 months of usage, there was a statistically significant difference in PLI among the four groups (P＜0.05). The difference of PLI between 6 months and 12 months in group D was statistically significant (P＜0.05). After 6 months and 12 months of usage of locator attachment implant overdenture, the difference of BOP positive detection rate among the four groups was statistically significant (P＜0.05). The difference of BOP positive detection rate between 6 months and 12 months of usage of denture in group D was statistically significant (P＜0.05).ConclusionWhen the thickness of gingival mucosa is＞3.0 mm, PLI and BOP positive detection rate in soft tissue around locator abutment are increased.
