摘要:目的探讨房间隔膨出瘤(atrial septal aneurysm,ASA)超声心动图参数特点。方法回顾性收集经胸超声心动图(transthoracic echocardiography,TTE)检查确诊为ASA的患者129例及无ASA的129例患者为对照组。获取患者的一般临床资料、有无卒中及心律失常;应用常规心动图检查获取左房前后径、升主动脉内径、左室舒张末期前后径,观察有无二尖瓣反流和主动脉瓣反流,有无合并先天性心脏病。比较两组的超声心动图参数差异,并采用二元logistic回归分析ASA发生的相关危险因素。结果二尖瓣反流和卵圆孔未闭(patent foramen ovale,PFO)发生率在ASA患者中显著高于对照组,差异有统计学意义(均P<0.001);ASA组较对照组升主动脉内径宽,差异有统计学意义(P<0.05);此外,ASA组患者主动脉瓣反流和房间隔缺损(atrial septal defect,ASD)的发生率也高于对照组,差异有统计学意义(均P<0.05)。二元logistic回归分析显示,二尖瓣反流、PFO和ASD是ASA发生的独立危险因素,均P<0.05。结论ASA患者的超声心动图表现普遍存在二尖瓣反流、主动脉瓣反流、PFO和ASD,ASA患者升主动脉内径较宽;且二尖瓣反流、PFO和ASD是ASA发生的独立危险因素。

Abstract:ObjectiveTo explore the echocardiographic parameters of atrial septal aneurysm (ASA).MethodsA retrospective study was performed on 129 ASA patients confirmed by transthoracic echocardiography (TTE) and 129 patients without ASA in the control group. The general clinical data and history of stroke and arrhythmia were obtained. Routine cardiography was used to measure the left atrial anterior and posterior diameter, ascending aorta diameter, and LV end-diastolic diameter, and to detect mitral regurgitation, aortic regurgitation, and congenital heart disease. The echocardiographic parameters were compared between the two groups, and binary logistic regression analysis was performed to determine the risk factors of ASA.ResultsCompared with the control group, the incidence of mitral regurgitation and patent foramen ovale (PFO) was significantly higher in the ASA group (bothP<0.001). The ascending aorta internal diameter was wider in the ASA group than the control group (P<0.05). In addition, the incidence of aortic regurgitation and atrial septal defect (ASD) was higher in the ASA group than the control group (P<0.05). Binary logistic regression analysis revealed that mitral regurgitation, PFO and ASD were independent risk factors for the development of ASA (allP<0.05).ConclusionMitral regurgitation, aortic regurgitation, PFO and ASD are commonly seen on echocardiography in patients with ASA. The ascending aortic internal diameter of ASA patients is wider; and mitral regurgitation, PFO and ASD are independent risk factors for the development of ASA.
