摘要:目的将拔除的自体牙制备为骨移植材料应用于牙槽骨植骨术，观察术后成骨效果及稳定性。 方法选择在大连医科大学附属第二医院口腔科就诊的3例需进行牙槽骨植骨术的患者，分别使用3种手术方式：牙槽骨引导骨再生术（GBR）水平骨增量术、上颌窦底内提升术和根尖囊肿摘除同期植骨术。术后即刻和定期随访软组织颜色和肿胀程度评价软组织愈合状况；CBCT或X线根尖片观察植骨区骨量和骨质密度的变化以评价骨再生效果。 结果牙槽骨GBR水平骨增量术患者种骨移植后新生骨骨密度增大，种植体未出现松动或脱落等症状；上颌窦底提升术患者术后3个月及14个月相较于术前，窦内新骨高度均增大，新生骨骨密度与术后即刻相比均增大；根尖囊肿摘除同期植骨术患者术后5个月较术后即刻骨密度灰度值增大。3例术后均无骨粉流失、感染溢脓等并发症。 结论自体牙来源骨移植材料应用于牙槽骨缺损骨移植术，可增加新骨形成量，加速骨再生，术后成骨效果稳定，可获得良好的临床效果。

Abstract:ObjectiveTo study the efficacy of osteogenesis and stability after applying extracted autologous teeth as bone graft materials in alveolar bone grafting.MethodsThree patients, who were admitted to the Department of Stomatology in the Second Hospital of Dalian Medical University and required alveolar bone grafting, were selected. Three surgical methods were used respectively: GBR horizontal bone augmentation for alveolar bone, osteotomy maxillary sinus floor elevation, and excision of periapical cyst followed by bone grafting. Soft tissue color and degree of swelling were recorded by clinical examination after the surgeries in order to evaluate the healing status of soft tissue. CBCT or periapical X-ray was used to observe the changes of bone mass and the bone mineral density in the bone grafting area in order to evaluate the effect of bone regeneration.ResultsIn the patient with GBR bone increment in alveolar bone, the new bone density increased after bone grafting, and there were no signs of loosening or detachment of the implant. For the patient undergoing maxillary sinus floor elevation surgery, the height of new bone in the sinus increased at 3 and 14 months after surgery, and the density of the new bone increased immediately after surgery. In the patient with apical cyst removal and bone grafting, the gray value of bone density increased at 5 months after surgery compared to that immediate after bone grafting. All 3 patients had no postoperative complications, such as bone powder loss, infection, and pus discharge.ConclusionThe application of autologous dental bone graft material in bone transplantation for alveolar bone defect can increase the volume of new bone formation, accelerate the process of bone regeneration, and the osteogenesis effect is stable, achieving good clinical results.
