摘要:目的检测Eotaxin-1、MCP-1和VEGF在膝关节骨关节炎患者血清及关节液中的变化并分析其临床意义。方法选择2019年1月至2020年12月在沈阳市骨科医院诊断为膝关节骨关节炎（KOA）患者30例为OA组，无KOA临床表征患者30例作为对照组，对比分析两组患者一般资料及临床症状，酶联免疫吸附试验检测血清及关节液中细胞因子Eotaxin-1、MCP-1和VEGF的表达情况。采用ROC曲线并计算曲线下面积及约登指数，确定各细胞因子的诊断能力；通过Logistic回归模型建立各细胞因子的联合指标，计算曲线下面积并确定各细胞因子联合指标的诊断能力。结果Eotaxin-1、MCP-1和VEGF在血清及关节液中的表达分别为73.91±18.87/46.16±12.35、78.64±19.61/259.45±46.57和92.89±30.23/165.72±35.55，与对照组比较均显著升高（P < 0.05）。ROC曲线分析显示，单独的细胞因子中MCP-1的诊断能力最高；联合指标Eotaxin-1+MCP-1和Eotaxin-1+VEGF+MCP-1的诊断能力最高。结论Eotaxin-1、MCP-1和VEGF在KOA的发病机制中具有重要的作用，血清联合指标Eotaxin-1+MCP-1可能对KOA早期诊断具有一定的参考价值。

Abstract:ObjectiveTo detect the changes of Eotaxin-1, MCP-1 and VEGF in serum and synovial fluid of patients with knee osteoarthritis and analyze their clinical significance.Methods30 patients diagnosed with knee osteoarthritis in our hospital from January 2019 to December 2020 were selected as experimental group and 30 patients without clinical signs of KOA were selected as control group, and the demographic data of the two groups was compared and analyzed, and the expression of cytokines Eotaxin-1, MCP-1 and VEGF in serum and synovial fluid were detected by ELISA. The diagnostic ability of each cytokine was analyzed by ROC curve and calculating the area under the curve and Yoden index, and the combined index of cytokine was established by Logistic regression model, the area under the curve was calculated and the diagnostic ability of the combined index of cytokines was determined.ResultsThe expression of cytokines Eotaxin-1, MCP-1 and VEGF in serum and synovial fluid was 73.91±18.87/46.16±12.35, 78.64±19.61/259.45±46.57, 92.89±30.23/165.72±35.55, significantly higher than control group(P < 0.05). The analysis of ROC curve showed that the diagnostic ability of MCP-1 was the highest among the single cytokines, and the diagnostic ability of combined indexes Eotaxin-1+MCP-1 and Eotaxin-1+VEGF+MCP-1 was the highest(P < 0.05).ConclusionsEotaxin-1, MCP-1和VEGF may play an important role in the pathogenesis of KOA, and the combined index Eotaxin-1+MCP-1 in serum may have a certain reference value for the early diagnosis of KOA.
