摘要:目的探讨α-Klotho和β-Klotho与妊娠期糖尿病(gestational diabetes mellitus,GDM)及妊娠不良结局的关系。 方法选取GDM患者(GDM组)和糖耐量正常妊娠者(NGT组)各66例,收集两组α-Klotho、β-Klotho、血糖、HbA1c、妊娠结局等指标并进行比较。采用Spearman线性相关分析法分析α-Klotho和β-Klotho与糖代谢指标的相关性,采用logistic回归分析法分析α-Klotho和β-Klotho与GDM及妊娠不良结局的关系。结果GDM组血清α-Klotho和β-Klotho水平低于NGT组(544.94 pg/mL vs 645.98 pg/mL;23.07 pg/mL vs 24.78 pg/mL,P<0.05)。α-Klotho、β-Klotho水平与FPG及HOMA-IR均呈负相关(P<0.05),与HOMA-β呈正相关(P<0.05)。GDM组更易出现妊娠期肥胖和巨大胎儿(P<0.05)。Logistic回归分析显示,经年龄、BMI、TC、TAG校正后,α-Klotho和β-Klotho均是GDM及巨大胎儿的保护因素(P<0.05)。结论GDM患者血清α-Klotho和β-Klotho水平降低,且二者均是GDM和巨大胎儿的保护因素,提示α-Klotho和β-Klotho可能参与了GDM及妊娠不良结局的发生过程。

Abstract:ObjectiveTo investigate the relationship of α-Klotho, β-Klotho with gestational diabetes mellitus and pregnancy adverse events.MethodsTotally 66 GDM pregnancies (GDM group) and 66 normal pregnancies (NGT group) were selected. The α-Klotho, β-Klotho, blood glucose, HbA1c and other indexes were measured and compared between groups. Spearman linear correlation was used to analyze the correlation between α-Klotho, β-Klotho and glucose metabolism indexes. Logistic regression was performed to analyze the relationship between α-Klotho, β-Klotho and GDM and pregnancy adverse outcomes.ResultsThe serum levels of α-Klotho and β-Klotho in GDM group were lower than those in NGT group (544.94 pg/mL vs 645.98 pg/mL, 23.07 pg/mL vs 24.78 pg/mL, P<0.05). The α-Klotho and β-Klotho levels were negatively correlated with FPG and HOMA-IR (P<0.05), while positively correlated with HOMA-β (P<0.05). GDM group more likely had obesity during pregnancy and macrosomia (P<0.05). Logistic regression analysis revealed that α-Klotho and β-Klotho were protective factors of GDM and macrosomia after correction of age, BMI, TC, TAG (P<0.05).ConclusionsThe serum levels of α-Klotho and β-Klotho in GDM pregnancies are decreased, and both of them are protective factors of GDM and macrosomia. It's suggested that α-Klotho and β-Klotho may play roles in the occurrence of GDM and pregnancy adverse events.
