摘要:卵巢子宫内膜异位囊肿（ovarian endometrioma，OMA）是子宫内膜异位症最常见的一种亚型，其具有进行性损害卵巢功能和术后易复发等恶性肿瘤样特性，是临床最关注的一种类型。OMA在育龄女性的发病率较高且常与不孕有关，但其导致生育力下降的机制仍不明确。OMA引起不孕的原因复杂，可能是多种机制并存。深入研究卵巢型子宫内膜异位症引发不孕的确切原因对促进卵巢功能保护、提高患者妊娠率具有重要意义。本文结合近年来国内外相关文献，就OMA引起不孕的可能机制作一综述。

Abstract:Ovarian endometrioma (OMA), the most common subtype of endometriosis, has attracted great clinical concern for its malignant tumor-like characteristics such as progressive damage to ovarian function and easy recurrence after surgery. The incidence of OMA is high in women of childbearing age and is often associated with infertility. However, the mechanism of OMA-related fertility decline is still unclear. The causes of OMA-related infertility are complex and may include many mechanisms. An intensive study of the exact mechanism of fertility reduction caused by ovarian endometriosis is of great significance to promote the protection of ovarian function and improve pregnancy rate. Combining with the relevant domestic and international literatures in recent years, this paper reviews the possible mechanisms of infertility caused by OMA.
