摘要:华法林是临床上常用的口服抗凝药物，常规用于血栓栓塞性疾病的防治。但是，华法林的治疗窗窄，不同种族及个体间剂量应用也存在较大差异，可引起出血或血栓栓塞等不良反应。近年来，国内外****一直在研究通过检测基因多态性来评估和调整华法林的合适剂量，并开发了一些给药算法模型用来指导华法林用药。本文主要围绕华法林的代谢、CYP2C9基因多态性对华法林剂量的影响、基因型指导的华法林给药算法进行综述，为临床合理使用华法林用药剂量提供参考。

Abstract:Warfarin is a commonly used oral anticoagulant in clinical practice and is routinely used in the prevention and treatment of thromboembolic diseases. However, the therapeutic window of warfarin is narrow, and the dosage of warfarin varies greatly among different individuals, which may cause adverse reactions such as bleeding or thromboembolism. In recent years, domestic and international researchers have been studying how to evaluate and adjust the appropriate dose of warfarin by detecting gene polymorphism, and some prediction models have been developed to guide warfarin dose. In this article, the metabolism of warfarin, effect of CYP2C9 gene polymorphism on warfarin dose and genotype-directed warfarin administration algorithm are reviewed to provide references for the rational use of warfarin dose in clinical practice.
