摘要:目的评价低强度激光疗法联合吸附重组牛碱性成纤维生长因子（rb-bFGF）的碘仿纱条对颌骨囊肿术后反应及组织愈合的临床疗效。方法选取2021年6月至2023年12月期间，被青岛市市立医院口腔医学中心诊断为颌骨囊肿并接受手术治疗的120例患者为研究对象，随机分为A组、B组、C组及D组，每组30例。A组患者术后使用单纯碘仿纱条填塞换药，B组患者术后给予术区低强度激光照射并使用单纯碘仿纱条填塞，C组患者术后使用吸附rb-bFGF的碘仿纱条填塞换药，D组患者术后给予低强度激光照射并使用吸附rb-bFGF的碘仿纱条填塞换药。使用视觉模拟量表（VAS）评分法对4组患者术后1、2、4、7天的疼痛程度进行评估，记录比较4组患者术后2、4、7天肿胀情况，对4组患者术后1周及2周换药时肉芽组织覆盖率进行评分。结果A组术后1、2、4天VAS评分显著高于B、C、D 3组，差异有统计学意义（P＜0.05）；B、C、D 3组间两两比较，差异均无统计学意义（P＞0.05）。D组术后2、4天肿胀程度明显轻于A、B、C组，B、C组肿胀程度均轻于A组，差异有统计学意义（P＜0.05）；B、C两组间差异无统计学意义（P＞0.05）。术后7天4组患者疼痛程度及肿胀程度比较，差异无显著性意义（P＞0.05）。D组术后7、14天肉芽组织覆盖率评分均高于A、B、C 3组，且C组高于A、B组，差异有统计学意义（P＜0.05），B组和A组间差异无统计学意义（P＞0.05）。结论低强度激光疗法联合吸附rb-bFGF的碘仿纱条相较于单纯碘仿纱条，可更大程度缓解颌骨囊肿术后疼痛，减轻术后肿胀，加快颌骨囊肿术后早期阶段愈合。

Abstract:ObjectiveTo evaluate the clinical effect of low-intensity laser therapy combined with iodoform gauze adsorbing recombinant bovine basic fibroblast growth factor (rb-bFGF) on the postoperative reaction and tissue healing of jaw cyst.MethodsFrom June 2021 to December 2023, 120 patients with jaw cyst who were diagnosed and underwent surgical treatment in the Dental Center of Qingdao Municipal Hospital were selected as the research objects. They were randomly divided into group A, group B, group C and group D, with 30 cases in each group. The patients in group A were treated with iodoform gauze filling after operation, the patients in group B were treated with iodoform gauze and low-intensity laser irradiation, the patients in group C were treated with rb-bFGF adsorbed iodoform gauze after operation, and the patients in group D were treated with low-intensity laser irradiation and rb-bFGF adsorbed iodoform gauze after operation. A visual analogue scale (VAS) was used in four groups of patients after 1, 2, 4, 7 days to evaluate the degree of pain. Postoperative swelling was compared among four groups after 2, 4, 7 days, and the coverage rate of granulation tissue was scored at the time of dressing change one week and two weeks after operation.ResultsThe VAS scores of group A were significantly higher than those of groups B, C and D at 1, 2 and 4 days after treatment (P＜0.05); while pairwise comparison among B, C, D three groups revealed no significant difference (P＞0.05). The degree of swelling in group D was significantly lower than that in groups A, B and C at 2 and 4 days after treatment (P＜0.05), and had no significant difference between groups B and C (P＞0.05). There were no statistically significant differences in the degrees of pian and swelling among the four groups at 7 days after treatment (P＞0.05). The scores of granulation tissue coverage in group D were higher than those in groups A, B and C at 7 days and 14 days, and the scores in group C were higher than those in groups A and B (P＜0.05); but there was no significant difference between group B and group A (P＞0.05).ConclusionLow intensity laser therapy combined with rb-bFGF adsorbed iodoform gauze can relieve the pain and swelling after surgery and accelerate the healing of jaw cyst in the early stage.
