摘要:目的比较调整缝线术与传统后徙术在间歇性外斜视（IXT）中的疗效。方法收集2020年12月至2022年6月于某战区医院眼科拟行外直肌后徙的IXT患者42例，按个人意愿及配合程度分为两组，A组22例行调整缝线术，B组20例行传统后徙术，术后随访6个月，分别对两组患者术后斜视度、术后眼痛、流泪、畏光、异物感、眼睑水肿、结膜充血消退时间进行比较。结果两组患者术前及术后1日、1周、1个月、3个月、6个月视近（33 cm）斜视度比较，差异均无统计学意义，P＞0.05；两组患者术前及术后1日、1周、1个月、3个月、6个月视远（6 m）斜视度比较，差异亦均无统计学意义，P＞0.05。两组术后第2日的眼痛程度、流泪、畏光、异物感、结膜充血消退时间比较，差异有统计学意义（P＜0.05），B组优于A组；而两组间眼睑水肿程度无明显差异（P＞0.05）。结论可调整缝线术与传统后徙术在IXT术后长期维持眼位方面效果一致，传统手术术后反应轻，患者舒适度好。

Abstract:ObjectiveTo compare adjustable sutures versus nonadjustable sutures in the treatment of intermittent exotropia.MethodsThe randomized prospective interventional study included 42 patients with intermittent exotropia, who underwent lateral rectus recession in our hospital from December 2020 to June 2022. The patient were divided into two groups: group A (n=22) received adjusted suture surgery, while group B (n=20) received traditional recession surgery. The postoperative follow-up was up to 6 months, and the postoperative strabismus, eye pain, tearing, photophobia, foreign object sensation, eyelid edema, and conjunctival congestion time were compared.ResultsThere were no statistically significant differences in deviation angle at 33 cm (near) between two groups before surgery and 1 day, 1 week, 1 month, 3 months, and 6 months after surgery (P＞0.05). There were no statistically significant differences in deviation angle at 6 m (distant) between two groups before surgery and 1 day, 1 week, 1 month, 3 months, and 6 months after surgery (P＞0.05) There were significant differences in eye pain, tearing, photophobia, foreign body sensation, and conjunctival congestion resolution time scores between two groups on postoperative day 2 (P＜0.05), group B was superior to group A. The degree of eyelid edema showed no statistically significant difference (P＞0.05).ConclusionAdjustable hang-back recession has the same effect for long-term eye position maintaining as traditional surgery. Traditional approach shows mild inflammatory stimulus response and high patient comfort after surgery.
