摘要： 目的 在CT引导下选取3种脉冲射频电压参数治疗带状疱疹后神经痛（PHN），观察其有效性和安全性。方法 选择胸段PHN患者118例，根据不同电压参数分为A组（电压45 V）、B组（电压65 V）、C组（电压85 V）。术后3 d、1个月、3个月、6个月、12个月时随访，观察患者治疗前后视觉模拟评分法（VAS）评分、健康调查量表36 （SF-36）生活质量评分以及并发症发生情况。结果 每个随访时间点3组患者的VAS评分、生活质量各项评分均优于术前（P < 0.01）。在长期随访中（3、6、12个月），B、C组患者的治疗效果明显优于A组（P < 0.05），并且术后6和12个月时，C组的VAS评分最低（P < 0.05）。术后3 d时B组和C组皮肤麻木面积较术前增大（P < 0.05），术后12个月3组皮肤麻木面积较术前减小（P < 0.05）。结论 提高脉冲射频电压参数（85 V）能提高对胸段PHN的治疗效果，且治疗安全、可靠。

Abstract: Objective To compare the efficacy and safety of CT-guided pulsed radiofrequency (PRF) at three different voltages for the treatment of postherpetic neuralgia (PHN). Methods This study included 118 patients with PHN involving a thoracic dermatome. Based on the PRF voltage used,the patients were allocated into group A (45 V),group B (65 V),or group C (85 V). After surgery,patients were followed up at 3 days,1 month,3 months,6 months,and 12 months. Observations at each follow-up included visual analogue scale (VAS) scores,36-Item Short Form Health Survey (SF-36) scores,and side effects. Results VAS scores decreased and SF-36 scores increased for all patients in the three groups at each post-operative time point (all P < 0.01). Pain relief and improvement in quality of life were more significant for patients in groups B and C than for those in group A at the 3-,6-,and 12-month follow-ups (all P < 0.05). Patients in group C had lower VAS scores than those in group B at 6 and 12 months (both P < 0.05). Patients in groups B and C had skin numbness affecting a larger area than before surgery at 3 days after the operation (both P < 0.05). All groups had skin numbness affecting a smaller area than before surgery at 12 months after the operation (all P < 0.05). Conclusion Compared with PFR at 45 and 65 V,PFR at 85 V had superior efficacy in treating PNH,with a favorable safety profile.
