二维斑点追踪成像技术评价慢性肺源性心脏病并发肺动脉高压患者右心室收缩功能

Abstract: Objective To evaluate right ventricular systolic function in chronic pulmonary heart disease (CPHD) patients with different degrees of pulmonary hypertension (PAH) using the two-dimensional speckle tracking imaging (2D-STI). Methods CPHD patients were collected from July 2016 to May 2020 in Shengjing Hospital of China Medical University. CPHD patients were divided into three groups according to differences in pulmonary artery systolic pressure (PASP). Patients with images of poor quality that could not be analyzed were excluded from each group, leading to the selection of 22 cases with good imaging quality from each group. Group A was the mild group (PASP ≥ 30-50 mmHg), group B was the moderate group (PASP ≥ 50-70 mmHg), and group C was the severe group (PASP ≥ 70 mmHg). Meanwhile, 26 healthy volunteers were selected to be part of control group. All participants were examined using 2D-STI. Longitudinal peak systolic strain (S) and peak systolic strain rate (SRs) were measured by 2D-STI in each group and segment. Differences between two-dimensional conventional parameters of ultrasound were compared. Results Compared with control group, the S of right ventricular free wall in group A, B, and C were decreased. The S of group A, B, and C decreased in turn. The difference between the groups was statistically significant (P < 0.05). Conclusion 2D-STI can sensitively and accurately evaluate the impairment of right ventricular function in patients with CPHD with varying degrees of pulmonary hypertension.
