Abstract: Objective To investigate the effects of different hypoglycemic therapies on the levels of acylated ghrelin (AG) and unacylated ghrelin (UAG) in newly-diagnosed type 2 diabetes mellitus patients, and analyze the correlation between the levels of AG and UAG and blood glucose or β cell function. Methods Sixteen patients were randomly divided into 3 groups and treated with exenatide (n=5),insulin (n=6),or pioglitazone (n=5) for 48 weeks. The levels of AG,UAG,insulin, and blood glucose were measured at fasting and at 30 minutes and 2 hours after a meal. Results The ratio of AG to UAG (AG/UAG) at fasting and 2 hours after a meal was significantly increased after hypoglycemic therapy (P<0.05). AG/UAG at fasting was negatively correlated with glycosylated hemoglobin A1c (r=-0.495,P=0.004). There was a negative correlation between the AG/UAG ratio and blood glucose levels at 2 hours after a meal (r=-0.393,P=0.026). Conclusion The increase in the AG/UAG ratio was associated with good glycemic control, but the change was not significantly different between the different hypoglycemic therapies.
Key words: newly-diagnosed type 2 diabetes mellitus, acylated ghrelin, unacylated ghrelin
