首发抑郁症患者认知行为治疗前后静息态脑功能磁共振低频振幅的变化

Abstract: Objective To further explore the neurobiological mechanisms,underlying the effect of cognitive behavioral therapy (CBT) in the treatment of first-episode patients with major depressive disorder (MDD),using amplitude of low-frequency fluctuations (ALFF) of functional magnetic resonance imaging (fMRI) in resting-state. Methods Eighteen first-episode patients with MDD and 40 healthy controls (HC),matched by gender,age,and years of education,were evaluated using fMRI scan. MDD subjects were treated only with an 8-week course of CBT and were re-evaluated afterwards using fMRI scan. MDD subjects (before and after treatment) and HC were assessed,using various scales. Every subject's ALFF was calculated by MATLAB-based Data Processing Assistant for Resting-State fMRI (DPARSF) software and then compared and analyzed. Results Compared with the results before CBT,the total scores of Hamilton Depression Scale-17 (HAMD-17),Hamilton Anxiety Scale (HAMA),patient health questionnaire-9(PHQ-9),and generalized anxiety disorder-7 (GAD-7) of patients with depression after CBT were significantly reduced,thus establishing statistically significant differences (P<0.05). After treatment,patients' ALFF value increased in the left middle frontal gyrus and decreased in the left middle occipital gyrus. ANOVA analysis showed that the difference between post-CBT and HC results was not statistically significant,suggesting that the brain area tended to normalize after treatment. Conclusion CBT can activate the activity intensity of the middle frontal gyrus and reduce the activity intensity of the middle occipital gyrus,thus exercising a two-way dynamic balance regulation effect on the brain function of patients with depression.
