甲状腺功能减退患者血清apelin表达水平的变化

摘要: 目的 检测甲状腺功能减退患者的血清apelin水平并探讨其与甲状腺功能和糖脂代谢指标的相关性。方法 选择确诊的临床甲状腺功能减退者、亚临床甲状腺功能减退者、单纯甲状腺过氧化物酶抗体阳性者各36例,另按年龄、性别匹配原则选择甲状腺功能正常者36例作为正常对照组。检测研究对象的血清apelin水平、甲状腺功能以及糖脂代谢指标等;分析血清apelin水平与甲状腺功能和糖脂代谢指标的相关性。结果 临床甲状腺功能减退组、亚临床甲状腺功能减退组、单纯甲状腺过氧化物酶抗体阳性组及正常对照组的血清apelin水平分别为(114.96±10.57)、(101.44±10.03)、(84.53±11.18)及(79.11±11.67)pg/L。与正常对照组相比,临床甲状腺功能减退组、亚临床甲状腺功能减退组血清apelin水平显著升高(P<0.01)。全部研究对象中,线性回归分析显示血清apelin水平与促甲状腺激素、甘油三酯水平呈正相关(r=0.405,P<0.01;r=0.438,P<0.01),与游离四碘甲状腺原氨酸水平呈负相关(r=-0.464,P<0.01)。结论 apelin可能参与了由高促甲状腺激素水平介导的糖脂代谢紊乱。

Abstract: Objective To determine the serum apelin level in patients with hypothyroidism and to investigate its correlation with thyroid function and glycolipid metabolism indexes. Methods This study included 36 patients with overt hypothyroidism,36 patients with subclinical hypothyroidism,and 36 patients that tested positive for isolated thyroid peroxidase antibody(TPOAb). The healthy control group consisted of 36 age- and gender-matched individuals,with normal thyroid function. The serum apelin level,thyroid function,and glycolipid metabolism indexes of all subjects were measured. The correlation between the serum apelin level,and thyroid function and glycolipid metabolism indexes was investigated. Results The levels of serum apelin for the overt hypothyroidism group,subclinical hypothyroidism group,isolated TPOAb group,and healthy control group were found to be 114.96±10.57,101.44±10.03,84.53±11.18, and 79.11±11.67 pg/L,respectively. The serum apelin level was significantly higher in the overt and subclinical hypothyroidism groups than in the healthy control group(P<0.01). Linear regression analysis indicated a positive correlation between serum apelin level,and thyroid-stimulating hormone and triglycerides(r=0.405,P<0.01;r=0.438,P<0.01),for all groups. A negative correlation was indicated between the serum apelin level and free thyroxine(r=-0.464,P<0.01). Conclusion Serum apelin may be associated with glycolipid metabolic disorders that are mediated by high level of serum thyroid-stimulating hormone.
