摘要Dysphagia is a common problem in patients with head and neck cancer but its importance as a complication of treatment for nasopharyngeal carcinoma is not well documented. Swallowing abnormalities are common in unselected patients following irradiation for nasopharyngeal carcinoma as detected by videofluoroscopy and symptom questionnaire. Patients with a history of nasopharyngeal carcinoma who were referred to a dysphagia clinic were reviewed. Thirty-two patients were seen in a 16 month period. All patients underwent a comprehensive swallowing assessment including fibreoptic endoscopic evaluation of swallowing (FEES), videofluoroscopy (VFSS) being performed in selected cases. Multiple abnormalities were common in the oral and pharyngeal phases of swallowing and affecting all food consistencies tested. Aspiration occurred in 52% of patients assessed by FEES and in 71% of patients undergoing videofluoroscopy. Of those patients aspirating during the VFSS examination, 58% did so without triggering a cough reflex (silent aspiration). The aetiology of dysphagia in postirradiation nasopharyngeal carcinoma is complex and patients often have severe swallowing abnormalities. Silent aspiration is common, and patients may be at risk of developing long-term pulmonary complications.
