摘要Cardiovascular diseases including coronary heart disease, stroke and hypertension are the far leading causes of death in Hong Kong. With the increasing in aging population from 12.8 % in 2http://aims.cuhk.edu.hk/converis/portal/Publication/0http://aims.cuhk.edu.hk/converis/portal/Publication/09 to 26.4% in 2http://aims.cuhk.edu.hk/converis/portal/Publication/036, the prevalence of cardiovascular disease is expected to go even higher. However, many of the risk factors for cardiovascular diseases, such as tobacco use, physical inactivity, and an unhealthy diet, are modifiable and can be corrected by lifestyle modifications. Yet, successful lifestyle changes require significant commitment by the individual and the ongoing support from the healthcare team. Chinese materials of hypertension, coronary heart disease and stroke are found to be scattered and many are far difficult for a layman to understand. Thus, empowering the general public by using information technology support is essential to promote cardiovascular health, as well as to confine the healthcare cost of Hong Kong. With the support of the Knowledge Transfer Project Fund from The Chinese University of Hong Kong, a new CVHealth website (www. cuhk.edu.hk/proj/CVHealth), is developed and launched for free public access. The website aims to empower the public to prevent and control the occurrence of coronary heart disease, stroke and hypertension. The website is supported by a multidisciplinary team of experts including cardiologists, pharmacologists, dieticians, physiotherapists, occupational therapists, medical social workers, as well as patient representatives. With the multiple local and national promotional strategies performed, this website has been well received by the Chinese living locally as well as living overseas. In this chapter, a series of knowledge transfer measures for preventing and managing cardiovascular diseases will be presented. Though empirical cardiovascular research and knowledge have been well established, transferring the developed knowledge, especially culturally-relevant to Chinese, to empower public to fight against these diseases is limited. ? 2http://aims.cuhk.edu.hk/converis/portal/Publication/012 Nova Science Publishers, Inc.
